Actilyse should be administered as soon as possible after the onset of thrombotic symptoms.
Myocardial Infarction
a) 90-minute (accelerated) administration regimen for patients with myocardial infarction whose treatment was started within 6 hours of the onset of symptoms: 15 mg IV bolus, 50 mg as an infusion over 30 minutes, then 35 mg infusion over 60 minutes to achieve a maximum dose of 100 mg.
For patients weighing less than 65 kg, the total dose should be calculated based on body weight: 15 mg IV bolus and 0.75 mg/kg body weight over 30 minutes (maximum 50 mg), then an infusion of 0.5 mg/kg over 60 minutes (maximum 35 mg).
b) A 3-hourly administration regimen for patients whose treatment was started within 6–12 hours after the onset of symptoms: 10 mg IV bolus, 50 mg as an IV drip infusion over the first hour, followed by an infusion of 10 mg every 30 minutes for the next 2 hours to reach a maximum dose of 100 mg over 3 hours.
For patients with a body weight of ?65 kg, the total dose should not exceed 1.5 mg/kg.
The maximum permissible dose of alteplase in acute myocardial infarction is 100 mg.
Adjunctive therapy
Concomitant antithrombotic therapy is recommended by current international guidelines for the treatment of patients with myocardial infarction and ST-segment elevation.
Pulmonary embolism
A total dose of 100 mg should be administered over 2 hours. The most common regimen used is: 10 mg IV jet stream over 1–2 minutes, 90 mg IV drip infusion over 2 hours.
For patients weighing ?65 kg, the total dose should not exceed 1.5 mg/kg.
Adjunctive therapy
After administration of Actilyse, heparin should be started (or continued) if the activated partial thromboplastin time (APTT) value is less than twice the upper limit of normal. The infusion should be adjusted accordingly to an APTT of 50–70 s (1.5–2.5 times the baseline).
Ischemic stroke
The recommended dose is 0.9 mg/kg (maximum 90 mg) administered by infusion over 60 minutes. 10% of the total dose is initially administered intravenously as a jet stream. Therapy should be initiated as early as possible within the first 3 hours after the onset of symptoms.
